This 25-minute tutoring lesson plan separates the starting work sample, a tutor model, guided practice and an independent exit. Its practice checker explains the noun reference, while the next-session note stays unresolved until an exit answer, reasoning and support record are supplied. The teacher key never fills those fields for you.

Start with the exact work sample
Label the opening example W01. Retain the question, the recorded answer “team” and the missing explanation. Ask, “Show me the word you answered about. What helped you choose team?” The reference answer for “it” is the box. The learner might have answered about “They,” but that remains a possibility to check. Write the learner's own explanation before choosing a response to the error.
Stanford's National Student Support Accelerator connects work samples and exit evidence to session planning and recommends open questions about unexpected errors. Use that approach without borrowing a diagnosis from a list of possible causes. The fictional example contains no real learner identity, class level or program requirement; supply those in your own program plan.
Teach with three different examples
Model M01: “Nia found a folder. She opened it.” Point to “it,” substitute “the folder,” and reread “Nia opened the folder.” Show how the object and action fit. “She” refers to Nia in this supplied sentence pair; the reference comes from the text, not an inference about a name.
Guided G01 changes the objects: “Jo picked up a cup. He washed it.” Ask for both references, then for the action that supports “cup.” The local practice check accepts “cup” or “the cup” for “it” and explains washing. Record any prompt separately. Independent E01 uses “Sam set a book on the table. Then Sam opened it.” Ask what “it” names and why “opened” matters. The nearby table is not the object being opened.
Give each part a time and an output
Budget 3 minutes for check-in, 4 for inspecting W01, 5 for modeling, 6 for guided practice, 4 for the independent exit and 3 for the next plan. The blocks total 25 minutes. Adjust actual timings to the learner rather than treating the schedule as a record of completed teaching.
Keep the model explanation out of the exit response box. The preview offers a separate teacher reference for checking your preparation. Its recognition of the word “book” does not assess the quality of a learner's explanation. A prompted answer stays marked as supported, even when it matches the reference.
Write a conditional handoff
After the exit, record the answer, explanation and support actually given. If the answer is missing, obtain evidence before extending. If the answer is correct but the explanation is absent, revisit the reasoning. If support was supplied, plan another independent check. An incorrect answer calls for a clarifying question about the strategy.
Only an independent answer with a meaningful explanation gives the tutor a basis to consider extending this task. The preview phrases that as a suggestion for teacher review, not an automatic mastery decision. Keep actual observations and work references in the tutoring program's required record; temporary screen entries are not saved learner records.

Create a complete original HTML example in US English for this task: Tutoring lesson plan template. Return one self-contained HTML file. Include all styles, data and JavaScript in the file. Use no remote dependencies. Return the working page, not an outline.
Use W01: The team packed the box. They carried it outside; recorded answer to “it” is team, explanation unknown. Model M01: Nia found a folder. She opened it; it means folder. Guided G01: Jo picked up a cup. He washed it; it means cup. Independent E01: Sam set a book on the table. Then Sam opened it; teacher reference book. Accept normalized “cup”/“the cup” and “book”/“the book”; other referents need another look. Explanations still require tutor review. Schedule 3/4/5/6/4/3 minutes, total 25.
Provide a guided answer box and a Check practice button with specific reference feedback. Keep a separate blank exit answer, explanation and a labeled radio group for Not recorded, Supported and Independent. Build a next-session suggestion only from those current fields: missing answer unresolved; correct answer without explanation revisit reasoning; prompted response needs independent check; independent correct response with explanation may support extension for tutor review. Do not grade prose or label mastery. A reset clears the actual-entry fields.
Use a page design suited to this particular task, readable text at least 16 px, generous spacing, visible keyboard focus and explicit labels. At 360 px, stack the content without page-level horizontal scrolling. Use type="button" click handlers for actions. Keep source facts and sample results distinct from actual observations. Leave unknown details visible rather than inventing them. Controls update only this temporary page; distinguish local changes from actual records. Provide concise task-specific feedback and a reset that clears entered answers and stale results.
Use the result
Before the next appointment, place the exit reference beside the proposed objective and check that the two agree. When reviewing a prepared HTML sequence in NOS with another tutor, you can change its displayed heading directly to identify the session. Keep the work sample and reasoning beside the next-session question so the handoff explains why that objective was chosen.
