A useful substitute teacher plans template links each scheduled block to an exact material, its location and the next step if it is unavailable. It also leaves room to record the actual stopping point. A planned lesson and a completed lesson need separate records.

Put the handoff details where they can be checked
Prepare a private packet with the teacher, class, date, revision and school-designated contact route. Do not guess any missing detail. In this original fictional example, the revision is R1, the date is TO CONFIRM, and the planned lesson block is 09:00–09:30.
Martin County School District's 2022–23 handbook discusses obtaining a substitute packet and schedule, reviewing materials, and leaving notes about accomplished and unfinished work. Those historical handoff concepts inform this packet; the district's forms and rules are not a current policy for your school. Substitute Teacher Handbook, 2022–23
Link to the actual school's authoritative packet for arrival, attendance, dismissal and emergency procedures. The location and contact route must be confirmed locally. This template does not invent those procedures.
Match every block to a material ID
Here is the proposed schedule. The times total 5 + 15 + 5 + 5 = 30 minutes, with no overlap:
| Planned time | Task | Required material | Prepared location or instruction |
|---|---|---|---|
| 09:00–09:05 | Teacher-provided warm-up | W01 | Printed copy, blue folder on front desk |
| 09:05–09:20 | Supplied practice task | P01 | Printed copy, same blue folder |
| 09:20–09:25 | Review the supplied task | K01 | Teacher-only key in the folder; do not distribute as learner work |
| 09:25–09:30 | Collect work | Work-collection tray | Use the labeled tray; confirm the physical label/location |
The IDs connect the plan to existing teacher-provided content. This example supplies no invented warm-up questions, practice exercises or answers. Before use, verify the actual copies and locations rather than assuming that a typed location proves availability.
Keep a preparation checklist with material ID, expected location, checked by, check time, actual availability and unresolved note. Leave real checker names, times and results blank until someone performs the check.
Make the missing-material branch explicit
Consider this separately labeled fictional walkthrough:
W01: present in the fictional scenario
P01: missing in the fictional scenario
K01: present, teacher-only
F01 fallback availability: unknown
F01 permission: unconfirmed
Practice coverage: unresolved
K01's presence does not repair the missing practice sheet. Nor does a fallback ID mean that the substitute has permission to use it. The handoff instruction should say:
If P01 is unavailable, do not invent replacement questions. Use F01 only when the teacher has preapproved it and its availability and permission are confirmed. Otherwise, use the school-designated office contact route, which must be confirmed in this packet, and follow the actual school's procedures. Record the unresolved material gap.
This branch does not establish that practice occurred. Even W01's fictional presence is not evidence that the warm-up was completed. Actual completion fields remain blank. If a fallback is later approved, document its material ID and the confirmed instruction; do not silently describe it as P01.
Leave a return note the teacher can act on
The returning teacher needs to know where to resume, not merely whether the substitute followed the printed timetable. Use this blank record for each block:
Date / class / packet revision: ______
Planned block: ______
Material actually used: ______
Actual stopping point: ______
Task or portion left incomplete: ______
Where the work was left: ______
Unresolved question for the teacher: ______
Recorded by / time: ______
For the fictional missing-P01 walkthrough, the useful question is: “P01 was unavailable and F01 permission was unconfirmed; which material and starting point should be used next?” Treat that as a sample note about a scenario, not an actual day's report.
Do not infer achievement from a collected worksheet or a scheduled review. Record what was used and where the activity stopped. Keep pupil names, health records, passwords and other confidential details out of any public presentation fixture. Maintain the real handoff in the school's authorized private record.
Copy this prompt for the handoff packet
Prepare an original US-English substitute handoff packet with a material checklist and blank return-to-teacher record. Use teacher-supplied lesson content only; do not invent exercises, school procedures or actual completion.
Fictional packet revision R1, date TO CONFIRM, planned block 09:00–09:30. Schedule: 09:00–09:05 W01 teacher-provided warm-up; 09:05–09:20 P01 supplied practice; 09:20–09:25 K01 teacher-only review key; 09:25–09:30 collect work in a labeled tray. W01/P01/K01 expected as printed materials in the blue folder on the front desk. Confirm the actual tray label/location. Durations 5+15+5+5 = 30 minutes, no overlap. Do not expose K01 as learner work.
Checklist fields: material ID, expected location, checked by, check time, actual availability and unresolved note. Actual checks and completion remain blank. Separate fictional walkthrough: W01 and K01 present; P01 missing; F01 availability unknown and permission unconfirmed; practice coverage unresolved. Presence is not completion.
Missing-P01 instruction: do not invent replacement questions. Use only teacher-preapproved F01 when availability and permission are confirmed. Otherwise contact the school-designated office route TO CONFIRM and follow actual school procedures. Do not claim practice proceeded. Any later authorized fallback needs its own material ID and confirmed instruction.
Blank return fields: date/class/revision, planned block, material actually used, actual stopping point, incomplete task, where work was left, teacher question, recorded by/time. Keep the fictional unresolved question separate from actual records; no guessed student achievement.
Add TO CONFIRM pointers to the actual school's arrival, attendance, dismissal and emergency procedures and designated contact route. Do not reproduce district forms or invent current rules, medical/discipline instructions or confidential pupil data. Maintain the native private packet as authoritative. A later HTML review may use only this sanitized fixture; controls and rendering checks remain pending. No data collection, notifications, printing/export, persistence or security guarantee.
Review the handoff before the absence
The local HTML example places the schedule, material checks and unresolved fallback question together for preparation review. NOS supports saving and rendering prepared HTML. Keep the school's maintained private packet authoritative and use only a sanitized fixture in a presentation copy.
Explore NOS to present the prepared handoff. The returning teacher should be able to identify the missing material and the next unresolved decision without mistaking the plan for work that was completed.
