“Extra time” on a classroom reference leaves important questions unanswered. Extra time for which task, how much, and under whose responsibility? An IEP at a glance template should keep those conditions beside the support so a quick summary does not change the instruction it summarizes.
Keep a route back to the maintained IEP beside every working summary. The original fictional example below pairs three supplied excerpts with a compact staff reference. A separate blank Markdown sheet lets you prepare the same structure in the format your school uses.

Start with the source and the intended reader
Identify the maintained plan, its relevant version, the staff member using the reference and where that person can access the complete document through the school's approved process. Keep the summary's preparation date separate from the plan's effective or review dates.
The IRIS Center's implementation guidance emphasizes access to the complete IEP, understanding implementation responsibilities, and knowing the supports each provider is responsible for delivering. A brief reference can help someone locate that information; it does not supply the full plan or demonstrate that a person has read it.
Use a permitted learner reference and only the identifying information needed in your actual school workflow. The teaching example contains no student name, diagnosis, school, signed plan or real educational record. Its document label, PX 0.1, identifies our authored example, not an approved IEP version.
Carry the situation, action and responsibility together
These three excerpts are invented solely for the formatting exercise. They are not recommended accommodations or instructions for an actual learner.
| Source | Original fictional excerpt |
|---|---|
| S1 | Independent reading practice: the classroom teacher provides written task directions and asks the learner to restate the first step before beginning. |
| S2 | Classroom writing assignments: the classroom teacher allows up to 10 additional minutes. This excerpt does not specify district or state testing supports. |
| S3 | Progress record: the case manager collects one dated classroom writing sample every Friday and reviews the weekly checklist. |
For each summary row, retain the situation, responsible role, exact action, timing or limit, and source reference. S1 needs both written directions and the first-step check before work starts. “Give help with reading” would discard those instructions.
S2 belongs to classroom writing assignments. Do not turn “up to 10 additional minutes” into “extra time on all tests.” That changes both the amount's precision and the scope. District or state testing information is not supplied in this excerpt; locate it in the maintained plan rather than filling the gap from a classroom row.
S3 assigns a different role and routine: the case manager collects one dated writing sample every Friday and reviews the weekly checklist. A summary should not move that responsibility to the classroom teacher or mark collection complete merely because the row exists.
Leave missing plan details visible
The filled reference leaves the actual learner reference, full-plan location, plan version, effective date, review date and contact route blank. Annual-goal wording, service details and assessment provisions beyond the supplied excerpts are also not provided. Do not invent them to fill a page.
If your local form includes goals or service information, copy the relevant approved wording and references from the maintained plan. Keep precise schedules, conditions and responsibilities intact. Ask the designated school contact about a conflict or unclear instruction; a summary-editing session is not a process for deciding new supports.
Keep implementation observations in their actual record. The student data binder explains organizing dated work and its measurement rules. This quick reference explains what the supplied instructions say; it does not establish that a support occurred or a learner progressed.
For separate classroom practice planning, the student goal-setting template organizes an agreed practice step and evidence for a check-in. That teaching plan is a different document from this reference to maintained IEP instructions. Do not import its sample goal, dates or support choices into the learner’s approved plan.
Check the short version against the long one
Read each summary row back alongside its source. Can the staff member identify when the instruction applies, what they need to do and where to check the complete wording? Ask a colleague to inspect the writing-time row and explain why it cannot answer the missing testing question.
When a maintained plan changes, compare the affected source passages and revise the corresponding reference rows. Record what version you used and when you checked it. Correcting a summary alone does not change the plan, notify staff or confirm implementation.
Prepare an IEP at-a-glance working reference from supplied plan text.
Approved school format and permitted learner reference: [ENTER]
Maintained plan/version, authorized location and relevant dates: [PASTE]
Intended staff reader and exact relevant excerpts with IDs: [PASTE]
Designated contact route and known unresolved questions: [PASTE]
For each row preserve the setting/task, responsible role, exact action,
timing/limits and source ID. Separate summary dates from plan dates.
Keep missing details blank and flag conflicting source instructions.
Do not infer testing provisions, goals, service minutes, diagnoses,
new supports, implementation, signatures or learning outcomes.
Return an editable reference and a separate blank scaffold. Add an
HTML reading copy only if requested, with complete self-contained source.
Maintain actual records in the school's approved environment. A fictional reference in NOS can help colleagues check the reading order and source links before adapting the maintained document. Finish by following one summary row back to the complete instruction, keeping its scope and unresolved questions visible.
