Parent communication log: keep the contact and its follow-up separate

Record what happened, who replied and what happens next.

Your note says “contacted home,” but the next teacher cannot tell whether anyone replied. Record the contact attempt, the response actually received, and the next action in separate fields. A message sent and a conversation completed are different entries in the story.

Use this original fictional example to check the format. The family identifiers, times, and arrangements are invented; no student or family record is being shared.

C01 — Monday, 3:10 p.m. The teacher sent a request to discuss the reading-folder routine through the agreed school channel. No response is recorded in this snapshot. Next action: the teacher checks the agreed channel Tuesday morning.

C02 — Tuesday, 8:20 a.m. The family replied to C01 and asked for the folder steps in the supplied home language. Next action: the teacher prepares the requested steps through the school’s agreed language-support arrangement.

C02 follows C01. It does not rewrite Monday's attempt as though the conversation had already happened. The example gives no actual calendar date or completion of the requested steps; those fields stay open.

A blank log can use these headings:

Entry ID:
Date/time and recorder:
Student/family identifier permitted in this record:
Purpose and agreed contact channel:
Contact attempt or conversation actually observed:
Family response, attributed to its source:
Agreed next action, responsible person, and follow-up date:
Related entry or maintained source record:

Write an observable description in the contact field. “Sent a message at 3:10 p.m.” describes an action; “family uninterested” adds an unsupported interpretation. Attribute a family's request to the response rather than turning it into a general statement about the child.

The Office of Head Start's family engagement standard describes respectful two-way communication and attention to a family's preferred language. That is the source's own program context. The blank log here is an authored working format, not a form supplied by Head Start or a statement that it satisfies your school's requirements.

Carry the next action into the next entry

Keep an unanswered attempt open until the record shows what happened next. A follow-up date tells the responsible person when to check; it does not prove the check was performed. When new information arrives, add a linked entry with its time and source.

If an error needs correction, preserve enough context to distinguish the correction from a later reply. Follow the institution's actual recordkeeping process. For a team discussion, use a permitted, limited excerpt rather than a public page containing family contact details.

The parent-teacher conference page tells families when and where to attend. This log records what happened during contact; keep it in the approved record location. Finish by assigning the actual follow-up to a person, then record the result when it occurs.
