Performance review meeting agenda template: keep evidence within the period

Four job aids were published in August and the fifth on October 2. For a September 30 goal, “five completed” loses the deadline.

Start the conversation with dated evidence inside the review period, then discuss later context separately.

The 35-minute agenda connects a fictional goal to the four publications recorded by period end and the later fifth. Its evidence filter changes the visible count and records while retaining the September result. The output is a discussion packet with open questions, not an employee rating.

Agree on the boundary before the conclusion

The review period is July 1 through September 30, 2026. G01 says, “Publish five reviewed job aids by September 30.” E01, dated August 20, records four published aids. E02, dated October 2, records the fifth. The associated review and quality evidence is not supplied.

The count supported at period end is 4 of 5 planned publications. E02 adds later context without moving its publication into September. Even 4/5 = 80% is a fraction of this publication target, not an overall employee performance score. Keep the goal's “reviewed” requirement unresolved until someone supplies the related evidence.

Prepare a packet both participants can inspect

MIT's performance-review guidance discusses preparation, agreed expectations, specific results and open issues. Use those conversation ideas while following the actual employer's private review process. Confirm identities, meeting details and evidence locations in that process rather than adding them to a public example.

Keep the original goal text and each dated record accessible to both participants. Ask, “What does E01 establish about publication, and which record would establish review?” That question names the gap without inventing a quality failure or an excuse for the timing. The later record should remain visible as context when discussing what happened after the deadline.

Use 35 minutes to produce specific notes

Allow 5 minutes to confirm the period and goals, 10 to inspect dated evidence, 10 to discuss obstacles and interpretations, 5 to consider next-goal options and 5 to review open issues. The total is 35 minutes. The example's agenda lengths are adjustable preparation choices, not an employer-mandated duration.

The local agenda builder lets you change those five lengths and calculates the new total and elapsed ranges. For instance, increasing evidence review from 10 to 15 produces 40 minutes overall. Give the extra time a purpose, such as locating a missing review reference, rather than padding every section with another summary.

Keep interpretations and agreements separate

Provide blank fields for the employee's view, the manager's view, the unanswered question and a proposed follow-up. These may differ. An AI-generated explanation of the dates cannot become either person's actual statement, and a combined summary is not proof that they reached agreement.

Publication count does not establish review quality. Neither filter state should generate a rating, salary recommendation, promotion or disciplinary conclusion. The preview displays the actual sample boundaries and questions; its temporary notes are not a private HR record or a persistence guarantee.

Leave with a traceable next question

A useful next-goal proposal is to define the next job-aid deliverable and the review evidence before setting its schedule. Keep owner, needed resources, due date and acceptance to confirm. Record actual agreement only when it happens under the employer's process.

At the end, read the supported factual statement aloud: four publications are recorded within the period, the fifth later, and review evidence remains missing. Then identify who will obtain the missing record and where the response belongs. Preserve unresolved interpretations rather than manufacturing a tidy consensus to fill the meeting notes.

Create a complete original HTML example in US English for this task: Performance review meeting agenda template. Return one self-contained HTML file. Include all styles, data and JavaScript in the file. Use no remote dependencies. Return the working page, not an outline.

Fictional period 2026-07-01 through 2026-09-30. G01 Publish 5 reviewed job aids by September 30. E01 Aug 20 records 4 published; E02 Oct 2 records fifth outside period. In-period 4/5 is 80% publication-count only, not a rating; review/quality evidence absent. Agenda 5 period+ 10 evidence+ 10 interpretations+ 5 options+ 5 open issues= 35 minutes.

Build an agenda sheet with the goal and dated evidence visible. Buttons filter in-period evidence or all context, computing visible publications while preserving the fixed period count 4/5. Editable five segment-minute inputs with Recalculate agenda must update elapsed ranges and total; reject blank, negative or fractional minutes. Include separate blank employee/manager interpretation and unresolved-follow-up notes, plus proposed next-goal owner/resources/date TO CONFIRM. Keep publication versus review evidence explicit. Reset returns 35 minutes and empty notes. Do not invent agreement, employee ranking, pay decisions, real identities, HR-system integration or saved private records.

Use a page design suited to this particular task, readable text at least 16 px, generous spacing, visible keyboard focus and explicit labels. At 360 px, stack the content without page-level horizontal scrolling. Use type="button" click handlers for actions. Keep source facts and sample results distinct from actual observations. Leave unknown details visible rather than inventing them. Controls update only this temporary page; distinguish local changes from actual records. Provide concise task-specific feedback and a reset that clears entered answers and stale results.

Open with the dated question

Begin the meeting with “Four publications by September 30; review evidence still needed.” For preparation, NOS can render the fictional HTML agenda with G01’s missing evidence beside the proposed time blocks. In the actual conversation, ask what would establish the review part of G01, then hear the employee’s and manager’s interpretations separately. Record any agreed evidence request with an owner and follow-up date in the employer’s private review form.
