Accomplishment Tracker: Save the Detail Before You Need the Review

Record the action, artifact and result while you can still retrieve the work.

You remember fixing a confusing handoff, but the review form asks what you changed and where the result can be found. An accomplishment tracker gives that work a dated entry while the details are still available. Record your action, the artifact and the supported outcome in separate fields.

Write the small factual entry now; draft the review answer later. This example is a running work log, with three original fictional entries and a blank format you can copy into your employer's permitted document or spreadsheet.

Make one entry useful on its own

A reusable entry has enough context to survive outside this week's conversation:

Field What to record
Work date and ID When the work happened and a stable reference for this entry
Need or problem The specific task you were addressing
My action What you personally produced, changed or checked
Evidence reference The permitted work record and version supporting the entry
Result so far What the evidence establishes, with unmeasured impact left open
Follow-up A question or next action, separate from completed work

Do not turn this into another list of adjectives. “Organized” is hard to retrieve as evidence. “Replaced the duplicate pickup instructions with one numbered checklist” tells you what to look for when reopening the work.

The Interior Business Center's closeout preparation guidance advises organizing notes, work examples, accomplishments and relevant feedback for formal discussions. That is the useful principle here. The agency's review dates, system instructions and number of required reviews do not become rules for another employer.

Preserve three different kinds of work

The fictional log's A01, dated October 6, records a revised pickup checklist. The employee combined two conflicting draft instructions into version P2. A teammate's note Q1 identifies the conflicting wording; P2 contains the replacement. The record supports a completed revision. It has no timing study or error-rate measurement.

A02, dated October 8, records a count check. The employee compared an original exercise list containing 14 entries with a list containing 13. One entry had been omitted, and a corrected list C2 restores it. “Located one omitted entry and restored the 14-entry list” matches the supplied record. “Eliminated reporting errors” would exceed it.

A03, dated October 13, records a draft explanation of the corrected list. D1 contains the explanation, but no reviewer response is supplied. The log keeps it under “Draft awaiting review.” It must not join the finished entries simply because a file exists.

Two recorded outputs are complete; one draft still awaits review. Those counts describe these three sample entries, not an employee performance rating. The example's filter switches between all three entries and the two completed outputs while leaving each contribution and limitation visible.

Capture the reference without copying private material

Use the source's actual title, version and permitted location. If the tracker should not contain the source text, store a reference that you can retrieve through the approved work system. Check the reference before the next discussion rather than assuming a filename identifies the right version.

For team work, name your contribution and preserve who did the rest. A record that you corrected one list does not make you the owner of the entire reporting process. Keep any colleague's actual feedback distinct from your own interpretation; a missing response is a question to follow up, not approval.

The ChatGPT performance review guide covers the later task of drafting answers against a review form. Start that task from selected tracker entries. Keep this maintained log as the dated source, rather than replacing it with a polished paragraph that loses the evidence references.

Copy this entry and prompt

Date / entry ID: __________
Need or problem: __________
My action and contribution boundary: __________
Source artifact / version / permitted location: __________
Supported result so far: __________
What remains unmeasured or unconfirmed: __________
Actual feedback, if received: __________
Next question or proposed follow-up: __________

Organize my supplied work notes into a running accomplishment log.
Preserve dates, source IDs, versions and my actual contribution.
Separate completed outputs, drafts awaiting review and future actions.
Do not invent impact, savings, praise, review agreement or ownership.
For each unsupported result, state the missing evidence in the log.
Keep confidential source content out of any wider reading copy.
Return the entries and unresolved questions, not a performance rating
or a submitted review. Actual employer format and private route: [ENTER].

Retrieve one entry before polishing it

At your next update, choose a real entry and open its source. If the result field outruns the evidence, correct the log before using the sentence elsewhere. Leave the next-action field open until the action actually occurs.

A prepared fictional HTML review copy in NOS can put the contribution beside its source excerpt. The example's filter changes only the current page; it does not save work notes, connect to a personnel system or send a review. Keep the maintained work log and your employer's required review process authoritative.
